What happens if I leave my employer and want to keep my primary care relationship with my selected provider group?

This depends on which provider group you select. Some groups will allow you to purchase a membership regardless of your employer. Others are independent clinics that partner with an advanced primary care network, so you can continue to see them if they’re in your network. However, they may not offer some of the specific benefits that were included with Aligned. Lastly, some provider groups, unfortunately, can only be accessed through a partnership with your employer or Aligned.
